ST. LOUIS -- Until now, eliminating undereye bags and hollows meant undergoing eyelid surgery, or “blepharoplasty.” Patients now have a new option.

Recent advances in facial aesthetics have allowed SLUCare facial plastic surgeon L. Mike Nayak, M.D. to use a new generation of soft tissue fillers to eliminate the appearance of baggy, sunken eyes with no incisions, no sutures, and no dressings.

Whereas first generation fillers, such as collagen, last for only a few months, the new fillers provide enduring improvements.

“There has been an incredible proliferation of soft tissue fillers over the last year,” said Nayak, the new chief of facial plastic and reconstructive surgery at St. Louis University School of Medicine. “We can now offer patients options that can last as long as two years.” These next-generation products are identical to natural components of healthy skin and bones, so they require no skin allergy testing.

Nayak said an aging lower eyelid has three components: bulging of fat in the eyelid itself, loss of fat along the rim of the eyebone (causing a groove or dark circles), and a flattening or drooping of the front of the cheek. Together, these changes create an unsightly series of bulges and valleys. “Think about an 18-year-old’s face,” Nayak said. “There’s a nice, smooth contour between the eyelid and the cheek.”

Men and women in their 30s and 40s begin to see the earliest signs of bags or hollows around the eyes, he said, and a 70-year-old might have a large bulge in the eyelid and deep trough just above the cheek.

“In the past, we’d go into the eyelid and take out the fat,” Nayak said. “This would decrease the contour difference between the lower lid and the cheek.”

For the right patient, doctors can now camouflage bags by filling in the trough, Nayak said, and accomplish a natural result nonsurgically.

“Plastic surgeons are just beginning to realize the importance of the lid-cheek transition. Together with the new generation of fillers, this new aesthetic is revolutionizing treatment of the lower lid.”

“For some people, I can hit a home run with this procedure. But it’s not right for everyone.”

The non-surgical eyelift consultation begins with a thorough evaluation and discussion of filler choices. The procedure itself is performed with topical numbing cream and takes minutes to perform.

“Patients can comfortably drive themselves home, and there is no need for antibiotics or pain medications,” Nayak said.

Using combinations of these techniques, he is able to achieve natural-looking, lasting results while minimizing downtime.
