The new fat autograft muscle injection (F.A.M.I.) technique, developed by French plastic surgeon, Roger E. Amar, MD, has shown improved long-term, aesthetic results for facial rejuvenation. In a recent study on 100 patients in the United States, clinical findings were reported and compared to those of traditional fat augmentation methods for facial volume restoration. Written by dermatologic surgeons Dr. Kimberly J. Butterwick of La Jolla, Calif, and Dr. Edward A. Lack of Chicago, Ill, the article, Facial Volume Restoration with the F.A.M.I. Technique, Preliminary Experience with a New Techniques, is due to be published in the October, 2003 issue of the American Society for Dermatological Surgery’s (ASDS) peer-reviewed Journal, Dermatology Surgery. In the article, Dr. Butterwick notes, “With the traditional method of fat grafting, longevity and symmetry have been unpredictable. However, the F.A.M.I. procedure appears to be a long-term, symmetrical, artistic procedure with high patient satisfaction.” The authors go on to comment, “The key to successful
rejuvenation of the aging face is restoring the fullness, firmness and resiliency
of youth.” As the face ages, volume diminishes. This loss of volume is a significant
part of the aging process and cannot be corrected with a rhytidectomy (facelift)
or laser resurfacing. “Many dermatologic surgeons consider fat the ideal filler
because it is natural, non-allergenic and readily available,” adds Dr. Butterwick.
For that reason, fat augmentation has long been a popular procedure, and most recently,
not simply placed under specific rhytids (wrinkles) and furrows, but for full face
volume restoration. The F.A.M.I. technique supposes two premises: that the volume lost during aging is not only fat, but involves atrophy of muscle and bone; and that transplanted fat survives best next to muscle. Several histological studies support this rationale. With the F.A.M.I. procedure, autologous adipose (fat) tissue is injected within the muscles of facial expression, thereby restoring fullness following the template of the patients’ own anatomy. In the experiment, fat was harvested in an atraumatic, sterile manner and the centrifuged fat was then injected into different muscle facial groups. Further, the injections parallel the blood supply,
minimizing trauma and optimizing Ideal candidates for this procedure are those with a gaunt look, hollowing eyes or flattened cheeks and lips. It is especially effective for post-facelift patients who wish to avoid the pulled appearance of multiple lifts. F.A.M.I. is also recommended for localized volume loss in areas such as the lips, tear trough, nasolabial folds and chin. Although questions regarding the longevity of all fat transplant methods including the F.A.M.I technique remain, these clinical findings demonstrate that the F.A.M.I. procedure provided a more predictable, long-term solution to age-related facial defects and successfully restored the smooth, attractive contours of youth.
