Nowadays you can’t always tell a person’s age by looking at his or her face, but in the past you could usually look at other telltale body areas like the hands, thighs or legs to calculate the years. Not so anymore…Today’s dermatologic surgeons who specialize in high-tech cosmetic surgery report they are developing new methods to beautify such body irregularities as cellulite, bulging veins, aging hands and stretch marks that traditionally belied a youthful appearance. Presenting at Beauty and Medicine: Transforming the Landscape of Dermatologic Surgery,” Dr. Neil Sadick, clinical professor of dermatology at Cornell University Medical College, showcased the latest anti-aging solutions to improve unsightly skin conditions associated with changes in body contour. He noted that one of the most exciting advances under investigation is a new technology for treating cellulite, the mottled, dimpled appearance of the skin, which has been resistant to previous therapies. Dr. Sadick told reporters here at the press briefing sponsored by the American Society for Dermatologic Surgery that “by combining infrared laser light and electrical conducted radio frequency (RF) energy to heat deep tissue with a mechanical suction action, we are able to melt and break up fat cell clusters.” Cellulite. Experts theorize that treatment works by producing three parallel effects: increased oxygen content for more efficient fat metabolism; increased blood flow and lymphatic drainage to stimulate evacuation of fat decay products; and physically disrupting or disassociating the fat intercellular connection. Preliminary studies suggest results occur after eight 30-minute treatment sessions. The combined mechanical vacuum and simultaneous infrared plus RF heat energy appears to cause a rapid homogenizing of the swollen fat cells and fluids trapped below the dermis that first brought about the “cottage cheese” appearance of the skin. This in turn releases the depressed anchor points between the layers of adipose tissue and increases the elasticity of the fibrous tissue, resulting in a marked improvement of the skin contour, explained Dr. Sadick, adding that the technology is not FDA-cleared in the U.S. Aging Hands. For aging hands, he recommends several different options that can be used separately or in combination. Prominent veins can best be managed with sclerotherapy and/or ambulatory phlebectomy techniques while skin tightening is best accomplished with the new radiofrequency technology. Dr. Sadick uses a longer wavelength laser plus a filler agent like one’s own fat to correct wrinkling of the hands and produce long-term plumping. Finally, for age spots and skin discoloration, short wavelength lasers and intense pulsed light technologies work well to achieve pigmentary normalization. Leg Veins. Innovative treatment modalities for effective removal of leg veins include foam sclerotherapy where a foam solution of a detergent-type sclerosing agent is used to treat both intermediate and large size leg veins more efficiently and in fewer treatment sessions. Newer long wavelength Nd:YAG lasers can now effectively treat leg vessels up to 4 mm in diameter with more predictably and in patients of various skin types. Endovascular technologies including radiofrequency and endovascular lasers have replaced conventional surgery for removal of the greater and lesser saphenous veins in a safe, cost effective, and no-downtime outpatient setting. These minimally invasive techniques rid patients of large damaged veins with excellent functional and cosmetic results. Glycerin is a new sclerosing solution that eradicates small spider veins with less inflammation in a single treatment session. Stretch Marks. New approaches to improve stretch marks include the infrared Nd:YAG lasers which stimulate production of new collagen in depressed stretch marks where a loss of elastic tissue and collagen play a major role. The newer intense pulsed light platforms and pulsed dye laser technologies are particularly suited to reducing the appearance of red stretch marks, while first-generation ultraviolet light sources stimulate melanocytes and enhance production of melanin to restore lost pigment in mature white stretch marks associated with pregnancy or rapid weight fluctuation.
